Revised Progress Report Templates

State Office of Department of Health and Ageing have updated the Progress Reporting template. These are attached for your use. The reporting template has been modified in two areas:
Page 2 - details in relation to organisation's accreditation status have been included.
Page 14 - the financial report includes an additional line to reflect the balance of funds in the project's bank account (please note, this is a requirement of the funding agreement)
