This Kit is adopts a novel approach to support general practitioners working with other professional and with people who experience comorbid mental health and substance use problems to develop combined care and business plans.. The PDF document attached utilises hyperlinks to link to additional online resources.
The Kit is also suitable for use by other community providers, such as allied health professionals and people receiving treatment.
It identifies the responsibility of all involved to ensure that the patient has a pivotal role in the treatment provided and provides patients with a genuine opportunity to be an active partner in treatment.
