Northern Division of General Practice seeks a Care Liaison Worker Part time position (.8)
A Care Liaison Worker is sought for a fixed term of 12 months to assist a small number of GPs in private practice in the Northern suburbs to provide care co-ordination for patients on opiate substitution pharmacotherapy. The worker will provide the GP information about available services for patients and will provide case support to patients, and liaise with the range of service providers regarding services required by individual patients according to their needs, by GP request. During the project, the worker will develop an information resource regarding available services for the client group.
